Helping to get our patients back to the comfort of their own homes with the support they need is the priority for us this winter. That’s why the Integrated Care system has established an Integrated CIOS reablement service, which started on Monday 8 January, with a dedicated on-site Integrated Reablement Team based at Royal Cornwall Hospital Treliske and UHP.

The team, which includes all of our reablement providers STEPS, the CHAOS Discharge Task Force and Humans – are working together as ‘one team’ and will be on hand to have early, guided conversations with patients before they become medically fit to help them get home faster and to reduce inappropriate referrals to community hospitals and into long term care home placements.

The team are running a ‘test and learn week’ as part of our winter plan energised week to make final tweaks to the way they work.
