Health and Wellbeing Coach

Work as part of a health and wellbeing team alongside GPs, Care Coordinators, other clinicians and Social Prescribing Link Workers, to provide an all-encompassing approach to personalised care. The Health and Wellbeing Coach will provide 1:1 personalised support to patients in the community who are referred to them by team members and local agencies.
The successful candidate will:
- Provide health and wellbeing coaching for the most vulnerable people in the community
- Have good communication, negotiation and people management skills and act with compassion and integrity
- Use coaching approaches, frameworks and models or other helping strategies, for example, motivational interviewing
- Have good understanding of mental ill health and emotional distress
- Work with a diverse range of people from different cultural and social backgrounds
- Work confidently and effectively in a varied and sometimes challenging environment
- Signpost and refer patients and support them to access suitable community provision and liaise with relevant agencies
- Accurately log, maintain and update patient records
- Have a full driving licence, access to and use of a vehicle
