Community Gateway Locality Lead – North Cornwall

The Community Gateway is an ICB (Integrated Care Board) funded, voluntary sector led coalition. Its purpose is to provide a simple point of access for everything the community has to offer in Cornwall. As an advisor, the Community Gateway Locality Lead will work as part of the Community Gateway to meet clients with complex needs within their community. Coaching conversations will draw on an individual’s strengths, to actively explore the self-management of their wellbeing within the context of their whole life needs. The aim is to reduce isolation and loneliness, prevent ill-health, and positively engage clients to achieve
personal goals.
