Advisor – North Cornwall

Pentreath are seeking an advisor to work as a Community Gateway Locality Lead across the North Cornwall area.

The Community Gateway is an Integrated Care Board funded, voluntary sector led project, and has a unique partnership of more than fifty Voluntary, Community and Social Enterprise organisations including Age UK Cornwall, Volunteer Cornwall, Chaos Group, Humans Cornwall, Cornwall Volunteer Sector Forum and Pentreath; its purpose is to provide a simple point of access for everything the community has to offer in Cornwall.

The advisor will:

• Work collaboratively with project partners to reduce isolation and loneliness, prevent ill-health, and positively engage individuals to achieve personal goals
• Meet individuals with complex needs who wish to improve their social situations and overall wellbeing
• Use guided conversations to enable clients to explore self-management of their wellbeing within the context of their whole life needs, and provide an appropriate package of support
• Manage their own caseload and use a Charity Log database to record client progression

A full driving licence and use of a vehicle for work purposes is an essential requirement
