We are looking for an inspired, experienced and flexible person to work with us on this exciting mental health and homelessness project.
About the Project
The project works with people who are temporarily homeless, or rough sleeping, to support them with their mental health needs to enable them to live fulfilling lives and achieve their hopes and ambitions.
Referrals will come from Health for Homeless, Cornwall Housing Ltd or other housing providers. This is a partnership project between Cornwall Housing, Cornwall Foundation Trust, Social Care, MIND, First Light and Pentreath.
About the role
The post holder will work collaboratively as part of a multi-disciplinary team, together with the client, the referrer and mental health professionals.
You will provide a wide range of interventions to assist clients with daily health and wellbeing needs, building resilience and improving wellbeing, connecting into community resources.
You will provide practical advice and support to clients, working within a recovery informed approach, taking a holistic view to support people’s recovery to allow them to reach and maintain a rich and meaningful life.
