Hospital and Home Team Lead – Mid-Cornwall

Hours: 24 hours per week. Working pattern to be agreed.
Salary: £15,784.22 per annum (£24,334 FTE)
Location: The role is primarily based at home with the requirement to work within Clients’ homes and occasionally at our Truro Head office or community hubs. The area to be covered is mid-Cornwall including Truro, although support to other areas may be occasionally required, e.g. Newquay, Bodmin, St. Austell Clays.

We are seeking someone with excellent communication, interpersonal and leadership skills, able to form positive working relationships with a wide range of people. Organised and accurate coupled with the IT skills to confidently record clients and referrals on to digital databases and systems, such as CharityLog and Strata. Finding opportunities for some of the most vulnerable in our community, to age well in their homes and communities.

Working within the hubs and across the community of mid-Cornwall, this role will support and encourage a small team of Hospital & Home Support Workers to empower and support older people out of hospital and back into their home and local community. You will work with the organisation to create a feeling of connectiveness with peer contact within your team, hosting team get togethers along with larger organisational meet ups. In this role, you will also seek to improve the quality and growth of the service, actioning compliments, issues and complaints from individuals, networking and promoting the service, anddeveloping and maintaining good working relationships with the community and local organisations with the aim of ageing well and living life better. Our charity is agile and flexible, we do whatever is needed for the right outcome at the right time. On occasion, the post holder will be required to be fully involved in the delivery of the service, completing assessments and providing cover within your area.
