Hospital and Home Coordinator – North & East Cornwall

Hours: 22 hours per week
Salary: £14,468.86 per annum (£24,334 FTE)
Location: Hybrid role, peripatetic, based in North and East Cornwall and offering outreach assessments within client’s homes.

This is an exciting opportunity to work across the Charitable and Voluntary Sector, making a difference to people’s lives in North and East Cornwall. We are seeking someone with excellent communication and interpersonal skills, able to form positive working relationships with a wide range of people. Organised and accurate coupled with the IT skills to confidently record clients and referrals on to digital databases and systems, such as CharityLog and Strata. Finding opportunities for some of the most vulnerable in our community, to age well in their homes and communities.

Working within the hubs and across the community of North and East Cornwall, this role will help empower and support older people out of hospital and back into their home and local community. In this role you will enhance the lives of older people by working with assessors and hub leads to coordinate their discharge support and seek opportunities to aid them in making good health and wellbeing choices and build good health and prevention messaging. The post holder will accept and allocate referrals from various partners, visiting and meeting clients in their home to carry out assessments, developing and maintaining good working relationships with the community and local organisations with the aim of ageing well and living life better.
