Re-writing the final chapter: living and dying well within our communities

The vision for Cornwall and Isles of Scilly is to ensure people and those close to them experience the best possible care and support towards, and at the end of their lives, informed by ‘what matters’ to them.

We live in an ageing population where increasingly, people are living with progressive, life-limiting illnesses. There is a range of vital services, projects and support provided to people in our communities from within the voluntary and not-for-profit sector, including support for those facing life-limiting, usually progressive illness, including conditions such as COPD, dementia, frailty, cancer etc. Living as well as possible until we die is something we all value. However, we know there are challenges in navigating and accessing the right care and support, at the right time – at all stages of life and illness.

This meeting is to invite Voluntary, Community and Social Enterprise (VCSE) sector colleagues to share experiences and insights, and shape how the lives of people in our communities draw to a close. It is an opportunity to inform the Integrated Care System plans for the immediate and longer term, build collaborative connections and establish next steps.

This work touches us all – and the lives of many within our communities. Through a facilitated, creative approach, and a focus on real-life stories and experiences, we invite you to participate in this interactive workshop.
